INSTRUCTIONS: This form is to be used by all individuals seeking public office within the United States of America to indicate both an understanding of their duties as declared by their oath of office and an understanding of how the office they seek is related to that oath. Completion of Section 1 is mandatory. Completion of Section 2 is optional.

SECTION 1 - FUNDAMENTAL GOVERNANCE

You are filing as a candidate for public office in the United States of America, thereby indicating your commitment to swear a legally binding oath to uphold and defend the US Constitution [5 U.S. Code 3331]. You are thereby obligated to declare a basic understanding of the oath and office you would discharge in the interest of the people whom you would serve.
